The hospital issues copies certified by an authorised officer, which can take several working days because the treating physician's approval is often required. The certified set is then translated and legalised as the destination requires. Request the complete set at once — discharge summary, laboratory results and medication list — to avoid a second application later.

Where the source is handwritten and unclear, the correct response is to ask the issuing institution for clarification, not to interpret it.
